Requirements of the Certified Nursing Assistant role:
● Licensed Nursing Assistant in the state of practice (CNA)

About the CNA role:
● Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care.
● Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner.
● Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life.
● Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments.
● Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care.
● Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts.
● Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse.
● Accurately document care provided in accordance with Care360 Hospice policies and procedures.
● Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences.
● Provide support and education to family members and caregivers within the scope of the CNA role.
● Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance.

Requirements of the Music Therapist role:
● Licensed Music Therapist (LPMT) valid in the state of Ohio.
● Bachelor’s or Master’s degree in Music Therapy from an accredited institution with active state licensure or certification as required by local regulations.
● Excellent interpersonal and communication skills, with the ability to build rapport with patients and families.
● Comfortable with travel to patient homes, nursing facilities, or hospice centers as required by the role.
● Strong understanding of the psychological, emotional, and physical aspects of end-of-life care.
● Functional ability to incorporate various musical instruments, musical and therapeutic modalities.
About the Music Therapist role:
● The Music Therapist will use music-based interventions to address the emotional, psychological, physical, and spiritual needs of patients on hospice services.
● Provide music therapy sessions to hospice patients in individual or group settings, based on the patient's specific needs and goals.
● Provides music therapy
● Use music as a therapeutic tool to help manage symptoms such as pain, anxiety, depression, and agitation, enhancing the quality of life for patients.
● Offer emotional support to families and caregivers through music, facilitating meaningful connections and moments of peace during difficult times.
● Conduct assessments of patients' physical, emotional, cognitive, and social needs to develop individualized care plans.
● Collaborate with hospice nurses, social workers, chaplains, and other team members to ensure a holistic approach to patient care.
● Offer grief support through music therapy for family members and caregivers before, during, and after the patient's death.
● Maintain detailed records (using EMR system) of music therapy interventions, progress, and patient responses in accordance with hospice care documentation standards.
● Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as needed.

About the RNCM role:
● Conduct comprehensive patient assessments, including vital signs and medical history review
● Travel to patients' residences to conduct visits
● Develop individualized care plans in collaboration with multidisciplinary teams
● Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.
● Perform evaluations for hospice care along with informational visits as needed.
● Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care.
● Educate patients and families on treatment plans, medication management, and health maintenance strategies
● Submit complete, accurate, and relevant clinical notes promptly.
● Participate in IDG and case conferences
● Performs on-call duties per policy and as needed
● Conducts an initial and ongoing comprehensive assessment of the patients
● Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care.
● Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed.
● Provide care plan to Hospice Aide and LPN as part of the IDG team.

● Observe and report changes in the patient's emotional or physical condition to the hospice nurse or care team.
● Support family caregivers by providing respite companionship when appropriate.
● Assist with light housekeeping related to the patient's immediate environment, such as tidying the room, washing dishes, or changing bed linens when needed.
● Encourage meaningful activities that align with the patient's interests, abilities, and preferences.
● Document visits and communicate appropriately with the interdisciplinary hospice team.
● Treat all patients and families with dignity, compassion, and cultural sensitivity.
● Must be able to provide genuine compassion for individuals facing serious illness and have respect for diverse cultural and spiritual beliefs.

● Verifies coverage of needed nurse aide visits by completing the daily schedules for nursing staff promptly, taking into account patients’ plans of care, patients and family needs, and geographic location, communicating the necessary information (daily itineraries) to the nurse and preparing for outside agency nurse aides as needed.
● Contact care providers and clients regarding day-to-day changes.
● Provides staffing plan for sick leave, vacation, and long-term leave.
● Produces reports and articulates interpretation to the administrator.
● Monitors callouts from the overnight administrator also receives calls directly from employees not reporting for work, logs details in all appropriate locations, including shift-specific call-out books and daily shift assignments.
● Works closely with the Payroll Department daily to ensure all staff time is recorded and transcribed to payroll accurately.
● Responsible for assuring patient/resident safety.
● Performs other duties as assigned.
