Requirements of the Hospice Scheduling Coordinator Role:
● Prior scheduling experience preferred
● Good computer skills, including but not limited to Google Office, MS Office/Excel.
● Experience using HomeCare HomeBase.
● Knowledge of basic medical terminology.
● Ability to articulate interpretation of reports.
● Excellent customer service, both internal and external.
● Organized with the proven ability to be efficient while multitasking.
About the Hospice Scheduling Coordinator role:
● Verifies coverage of needed nurse aide visits by completing the daily schedules for nursing staff promptly, taking into account patients’ plans of care, patients and family needs, and geographic location, communicating the necessary information (daily itineraries) to the nurse and preparing for outside agency nurse aides as needed.
● Contact care providers and clients regarding day-to-day changes.
● Provides staffing plan for sick leave, vacation, long term leave.
● Produces reports and articulates interpretation to the administrator.
● Monitors callouts from the overnight administrator also receives calls directly from employees not reporting for work, logs details in all appropriate locations, including shift-specific call-out books and daily shift assignments.
● Works closely to Payroll Department daily to ensure all staff time recorded and transcribed to payroll accurately.
● Responsible for assuring patient/resident safety.
● Performs other duties as assigned.

Requirements of the Intake Coordinator role:
● Licensed LPN in state of practice
● BLS Certified
● Experience in home health/hospice care preferred
● Must be computer literate, able to maintain simple records
● Must be able to communicate effectively orally and in writing
About the Intake Coordinator role:
● Answer phones, and receive all referral/intake information.
● Complete referral/intake forms and route them appropriately for admission approval for all Care360 Hospice locations
● Assist in the scheduling of appropriately skilled professionals to fill the case, and input scheduling information into the computer system.
● Check common working files for Medicare Coverage and verification of the benefit period.
● Provide an explanation of the benefits to patients, families, and community contacts
● Assist community outreach representatives with marketing activities to promote relations with referral sources
● Adhere to work schedule and maintain good punctuality and attendance record
● Provide telephone and clerical support to other staff members as time allows
● Assist with maintaining office records and printouts.
● Perform other duties as directed

About the Admissions Nurse role:
● Provide coordination and completion of admissions in a timely manner.
● Conduct comprehensive physical and psychosocial assessments of potential hospice patients to determine eligibility for hospice care, including reviewing medical history, current symptoms, and prognosis.
● Assist by facilitating freedom of choice, relief of suffering, and passing with dignity for patients in end-of-life stages.
● Assess patients to determine if Hospice care is appropriate using the LCD’s federal and state regulations, hospice policy and procedures, and clinical knowledge based on guidelines.
● Professionals must be able to cooperate with various disciplines, including social workers, chaplains, Nursing Aides, and Volunteers.
● Assess and coordinate a pain/symptom management program formulated with the participation of family members and patients, utilizing pharmacological and non-pharmacological methods of pain/ symptom control to enhance quality of life.
● Provides hands-on nursing care, interacts with patients and their loved ones, all interdisciplinary team members, and other healthcare professionals involved with patient care.
● Provide support and education for the patient and their families based on the Hospice philosophy of care.
● Report to DOCS/ED and the medical director on the patient's condition and status of admission.
● Have a strong desire to learn and demonstrate an aptitude for the hospice philosophy and comfort care.
